Ventricular pseudoaneurysm. Rare but potentially fatal complication of acute myocardial infarction

A 75-year-old male patient in cardiogenic shock due myocardial infarction and severe mitral regurgitation secondary to posteromedial papillary muscle rupture was referred to emergent surgery for mitral valve replacement. The control echo showed a cavity was evidenced in the inferior wall of the heart, compatible with pseudoaneurysm.
Echocardigraphy is the key technique for the evaluation of myocardial infarction mechanical complications. However, multimodality imaging can also be useful in selected cases when more information is needed.
