We present the case of a 77-year-old woman with a history of mitral and aortic valve replacement with mechanical prostheses, who developed severe tricuspid regurgitation and right heart failure during follow-up. Due to the high surgical risk and the fact that the tricuspid valve had unfavorable anatomical characteristics for edge-to-edge repair, percutaneous orthotopic tricuspid valve replacement was performed with a Lux-Valve Plus prosthesis, guided by transesophageal echocardiography. The procedure was successfully completed, achieving precise valve positioning without complications. This case highlights the importance of transesophageal echocardiography for procedure selection and monitoring.
