Mitral annular calcification is a common condition, frequently affecting elderly women and advanced chronic
kidney disease patients. However, caseous mitral annular calcification (CMAC) is less common, mainly associated
with chronic hyperparathiroidism and calcium hemostasis abnormalities. We present a case of multiple dystrophic
cardiac calcification, including CMAC, and its evolution after kidney transplant
