We present a case of a 44-year-old female bodybuilder at a semi-professional level whose only cardiovascular risk factor is the use of various anabolic steroids. She was admitted due to hemoptysis, increased volume in the right leg, and dyspnea. During her hospitalization, we identify thrombosis at her right leg, in both ventricles, and pulmonary thromboembolism, as well as diastolic and systolic dysfunction of the left ventricle. Management with vitamin K antagonist anticoagulants was provided, with which she presents upper gastrointestinal bleeding and a transient ischemic attack.
