Presentamos el caso de un varón de 61 años, quien fue referido a nuestra unidad bajo diagnóstico de ruptura septal ventricular como complicación de un infarto agudo de miocardio con elevación del ST anterior. Fue ingresado en la Unidad de Cuidados Coronarios, donde se le realizó un ultrasonido transtorácico y transesofágico en el que se detectó una comunicación interventricular septal apical. Este caso destaca la importancia del ultrasonido en el infarto y del diagnóstico temprano de sus complicaciones
