Prosthetic valve thrombosis remains a frequent and fatal complication. It is more associated with mechanical prostheses and in mitral position, increasing the risk of complications in patients with poor adherence to anticoagulant treatment. We present the case of a patient with mechanical mitral and aortic prostheses, who presented data of heart failure after suspension of anticoagulation, showing a large left atrial thrombus, which led to a fatal outcome. The objective of this case is to know prosthetic valve thrombosis as a feared complication and the relevance of transesophageal echocardiography to provide urgent diagnosis and treatment.
