Abstract

We present the case of a 60-year-old woman with a history of hypertension and permanent atrial fibrillation referred to Cardiology outpatient clinic after being hospitalized for heart failure. The transthoracic echocardiogram showed a moderately dilated right ventricle with normal systolic function and mild pulmonary hypertension. The transoesophageal echocardiogram revealed  a valve-incompetent patent foramen ovale with a left-right shunt that had a functional behaviour of an atrial septal defect. Estimated Qp/Qs was 1.8 hence percutaneous closure was proposed and successfully performed.
