80 year-old patient with an incidental finding of thrombotic mitral valve disease. Prosthetic mitral endocarditis probably as a complication of the previous finding (B. hanselae). This cause is unique due to the unusual clinical presentation and the cause of the infection. The new multimodality imaging techniques were a key factor to establish a correct diagnosis.
