We are pleased to present the April 2026 issue of RETIC.

This issue opens with an editorial titled “Mitral Annular Disjunction: When a True Observation Becomes a Conceptual Error,” which invites reflection on the critical interpretation of cardiovascular imaging findings in the context of a clinical condition that is as fascinating as it is complex.

The issue includes two highly relevant review articles. The first addresses the brain-heart syndrome from a comprehensive approach based on multimodal cardiac imaging, for differential diagnosis and clinical management. The second focuses on the assessment of systemic venous congestion using transesophageal echocardiography (T-VExUS), describing its fundamentals and applications in critically ill patients.

The issue is rounded out with ten clinical cases, selected for their educational value and diagnostic complexity. Among them are conditions such as massive pericardial effusion, complex valvular complications, unusual coronary pathologies, endocarditis, and other clinical presentations where multimodal imaging plays a key role.
