Infective endocarditis is a complication of high-flow congenital heart defects; The presence of septal defects confers a 20 to 30 times higher risk of endocarditis, compared to the general population. This complication determines an increase in morbidity and mortality. The case of a pediatric patient is presented, where the initial diagnosis was by means of a transthoracic echocardiographic study. The evaluation of fever of origin to be determined, having a history of uncorrected high-flow congenital heart disease, in this case ventricular septal defect, the main suspicion is infective endocarditis.
