We present the case of a 21-year-old man under follow-up by pediatric cardiology after finding a mitral valve with a double orifice that was functionally normal and with preserved systolic function. In this case, the diagnosis of double orifice mitral valve is an incidental echocardiographic finding that requires a high degree of suspicion and professionals trained in congenital heart disease for its diagnosis.
