Abstract

We present a 50-year-old male with two-month history of constitutional syndrome diagnosed with subacute infective endocarditis due to susceptible Streptococcus sanguinis. Echocardiography revealed a bicuspid aortic valve with severe stenosis and regurgitation, a 13mm vegetation, a second vegetation in the mitral-aortic intervalvular fibrosa, abscess, and pseudoaneurysm. The CT scan was crucial in order to identify two peripheral embolisms and to anatomically define the perivalvular extension of the disease for the surgical planning. The patient underwent urgent surgery and subsequently developed complete atrioventricular block that required pacemaker implantation.
