Echocardiography is diagnostic method of cardiology with multiple advances since its discovery. It has allowed to make more accurate diagnosis, to perform more reliable surgical plans and to correct medical treatment. The following report is about a patient with an unicuspid aortic valve, in which 3D echocardiography played a fundamental role in the assessment of valvular morphology. Despite being an exceedingly rare valvulopathy, it is important to consider it in the study of the patient with aortic valve dysfunction
