Abstract

An 84-year-old woman with a history of mitral and aortic valve replacement was diagnosed with a mass on the pulmonary valve during a routine check-up. Transthoracic echocardiography revealed a mobile nodular mass attached to the pulmonary valve. Transesophageal echocardiography defined its morphology, mobility and attachment site, while cardiac computed tomography confirmed its valvular origin and polypoid appearance. Findings were consistent with pulmonary papillary fibroelastoma, an exceptionally rare location for this tumor. A sequential multimodality imaging approach enabled a confident diagnosis and guided conservative management tailored to the patient’s clinical profile. This case underscores the value of integrating complementary imaging techniques for the assessment of uncommon valvular lesions and for informed therapeutic decision-making.
