We present the case of a 75 year-old woman who is sent to our hospital for cardiac surgery. The surgery goes without complications but during the post-operative period the patient presents haemodynamic instability with clinical data that suggests cardiac tamponade, so an echocardiogram is performed. This image technique rejects pericardial effusion and, at the same time, gives the pieces of information needed for the diagnosis of this unusual entity. It is a iatrogenic dissection of the interventricular septum, a very unusual complication, with a very complex treatment and a gloomy prognosis.
