Intramyocardial dissecting hematoma is a rare but serious complication of acute myocardial infarction. It occurs due to rupture of the intramyocardial vessels and dissection of the myocardial planes extending through the interstitial tissue, which may eventually cause a communication with the ventricular cavity or lead to a free wall or septal rupture depending on its location. Its high mortality requires early clinical and imaging diagnosis. This clinical case is an example of the usefulness of echocardiography in decision making in this pathology
