Intramyocardial dissection (ID) is an uncommon complication of acute myocardial infarction, associated with late revascularization. However, it carries a high mortality rate, requiring timely diagnosis and individualized treatment, based on the hemodynamic status and extension of dissection. Is important to diagnose it using multimodal imaging. Among them, echocardiogram and magnetic resonance imaging are beneficial, the second with a better spatial resolution that allows better defect distinction. Therapeutic management guidelines have not yet been standardized for this pathology. The management of ID depends on multiple factors, in some cases, may be managed conservatively.
