Based on a case of a male patient, treated in emergency for syncope, with coronary heart disease suspected, in whom coronary angiography did not show significant angiographic lesions, but flow abnormalities suggestive of anomalous coronary origin; Coronary angiotomography was performed, which confirmed the anomalous origin of the left coronary trunk in the pulmonary artery; A review of this rare pathology is performed
