We present a case of Marfan syndrome with severe aortic insufficiency in which mitral annulus disjunction is demonstrated. Mitral annulus disjunction (MAD) is characterized by a displacement between the junction of the annulus with the ventricular myocardium. The significance of this abnormality is its association with ventricular arrhythmias and sudden cardiac death. There is a relationship between the size of the disjunction with the risk of ventricular tachycardia; it has been identified that the greater the displacement, the greater the risk of sudden cardiac death.
