Thrombosed giant right coronary artery aneurysm

Giant coronary aneurysms are an extremely rare entity (affecting approximately 0.02% of the general population). Although the most frequent cause is atherosclerosis, their aetiology is quite diverse (including vasculitis, and connective tissue diseases). Clinically are frequently asymptomatic, being diagnosed as an incidental finding while studying other conditions. However, in uncommon cases, they can also pose serious risk, presenting as an acute coronary syndrome, as a pericardial tamponade, or even as a sudden death. Due to the lack of evidence, their management is still controversial, and is crucial to individualise the treatment to patient and aneurysm characteristics.
