Introduction: Chronic thromboembolic pulmonary hypertension is a potentially curable variant of pulmonary hypertension. Non-invasive imaging techniques are essential for diagnosis and follow-up. The delay in diagnosis leads to functional cardiopulmonary damage, beyond the thrombotic obstruction itself. Clinical case: Patient with progressive dyspnea and heart failure, without previous known pulmonary thromboembolism, who shows in images a diagnosis of chronic pulmonary thromboembolism with signs of pulmonary hypertension, associated with aneurysm of the pulmonary artery and vascular compromise of neighboring structures.
