The clinical presentation of aortitis is highly variable, ranging from chest pain to an asymptomatic aortic aneurysm detected incidentally. It can sometimes mimic acute aortic syndrome (AAS). Given that the therapeutic approach is very different, the differential diagnosis is of great importance. We present the case of a patient with chronic aortitis who underwent aortic replacement surgery due to suspected aortic intramural haematoma (AIH), which serves as a good example illustrating the usefulness of multimodal imaging techniques in uncertain cases.
