Obstructive hypertrophic cardiomyopathy, owing to the turbulent flow it generates, is one of the predisposing factors for infective endocarditis. The morbidity and mortality associated with this condition necessitate prompt diagnosis and treatment, where echocardiography plays a pivotal role. Herein, we present a case of a patient with hy-pertrophic cardiomyopathy who develops mitral endocarditis secondary to a pseudo-aneurysm over the A2 scallop, complicated by vasculitis, severe pericardial effusion, and valvular destruction with severe mitral insufficiency requiring valvular replacement surgery.
