Imaging diagnosis of prosthetic valve endocarditis often represents a diagnostic challenge, with echocardiographic criteria that have evolved, the most recently proposed being prosthetic valvulitis. We present the case of a 72-year-old man, hospitalized 4 months after biological aortic valve replacement with subacute febrile syndrome and positive blood cultures for methicillin-resistant Staphylococcus haemolyticus, in whom transesophageal echocardiography and later PET CT, demonstrate isolated prosthetic valvulitis. A brief review of the most frequent characteristics of this presentation of prosthetic endocarditis is made
