Pulmonary thromboembolism after left atrial appendage closure: how did the thrombus get there?

Left atrial appendage percutaneous closure is an increasingly expanded treatment to avoid anticoagulation in patients with atrial fibrillation and high bleeding risk. However, in a small percentage of patients, thrombosis may occur on the surface of the device. The fundamental tools to characterize the anatomy and path of possible thrombi are echocardiography, especially transesophageal, and computed tomography (CT) with contrast. In this case, we present a patient with a device thrombosis who also simultaneously presented a paradoxical pulmonary thromboembolism from this thrombus in the left atrium, which diagnosis was performed by cardiac CT.
