Left main coronary artery agenesia, and critical bicuspid aortic stenosis. More than a murmur.

We present the case of a 56-year-old patient who comes to her medical check-up reporting episodes of dyspnea in functional class II-III and upon auscultation a systolic murmur is detected. A transthoracic doppler echocardiogram was performed, which confirmed the presence of critical aortic stenosis. In the presence of non-specific symptoms, a coronary angiography is requested in order to evaluate the probable ischemic etiology of the symptoms, which reveals the presence of a complex coronary anomaly and the absence of angiographically significant lesions. For a more exhaustive evaluation of coronary and aortic pathology, a multi-image evaluationis performed.
