Percutaneous edge-to-edge repair in complex mitral regurgitation, is it really a contraindication?

Transcatheter edge to edge repair (TEER) represents an effective alternative to valve repair surgery in patients with severe primary mitral regurgitation in whom surgical risk is unacceptable. Adequate prior echocardiographic study is essential for the success of the therapy. Different prognostic scores and criteria have been published to guide the patient selection. We report the case of a 77-year-old woman admitted to our hospital with acutely decompensated heart failure in whom a severe primary mitral regurgitation with a complex valvular anatomy was effectively treated by TEER.
