Multiple coronary fistulas. Revision after diagnosis of two cases

Abstract

We present 2 cases of multiple coronary fistulas, which were suspected by color doppler images obtained at the beginning of a stres test, in an Echocardiogram with coronary blood flow,settings. The first case during preoperative workup for vascular surgery, and the second whithin  chest pain evaluation. In both cases, the presence of these fistulas are confirmed, arising from the 3 coronary branches and draining into the left ventricle cavity. Literature review indicates that this type of fistulas ocurre very infrequently, and are almost always unsuspected findings in angiographic studies. The majority of published cases have a benign prognosis, however in some cases the presence of myocardial ischemia has been attributed to coronary steal phenomenon. For the unusual presence symptoms, beta-blocker have been used.
