Complex fistula with connection of aorta and left coronary tree with pulmonary artery

We report the case of a 72-year-old female who was referred to the cardiology department after casual detection of a coronary artery malformation in a thoracic CT. Having performed an echocardiography, coronary CT and cardiac catheterization, we reached the final diagnosis of a complex fistula which connected right sinus of Valsalva and left coronary artery with the main pulmonary artery. As the patient was asymptomatic and did not show cardiac repercussion, it was decided not to close the fistula and continue with clinical and echocardiographic follow-up.
