Abstract

We introduce the case of a patient with a dual-chamber permanent pacemaker who was admitted because of an initial diagnosis of pulmonary thromboembolism. In the CT scan a filling defect in the right atrium (RA) was detected. A transthoracic echocardiogram was performed and a large mobile mass was observed in the RA attached to the atrial pacemaker lead compatible with thrombus due to its characteristics and location. The presence of antiphospholipid syndrome was detected as a predisposing prothrombotic cause. Medical treatment with indefinite
anticoagulation was chosen due to the clinical stability of the patient.
