We present a case of 40-year-old male patient with a history of ischemic heart disease, right ventricular thrombus and systolic dysfunction. We report the case after successful chronic therapy with oral anticoagulation. After 2 years of follow-up, the patient remains stable without presenting data of pulmonary embolism, heart failure, arrhythmias, bleeding or the need for invasive management, without remission of the thrombus. The current literature does not show a consensus on the ideal treatment in these patients, an integral evaluation of risk-benefits is required, together with the experience of each center in the therapeutic option used.
