Right atrial fibrosis and sick node syndrome

A 57-year-old hypertensive and obese male, consulted for assessment due to dyspnea on exertion. Mild pulmonary
hypertension due to diastolic dysfunction without ventricular hypertrophy, chronotropic incompetence and right atrial fibrosis were detected. There was also deterioration of the strain of the right atrium. A permanent pacemaker was implanted. In this case we will discuss how, underlying the appearance of automatism disorders and atrial arrhythmias, there may be a non-ischemic fibrosis substratum.
