We present an 81-year-old male admitted for the study of fever associated with weight loss, asthenia, and decrease of appetite. The transoesophageal echocardiogram confirmed the existence of a complicated mitroaortic endocarditis with an aneurysm and perforation of the mitral anterior leaflet.
