We present the case of a 33-year-old male with a history of bicuspid aortic valve with moderate regurgitation who was admitted in the emergency room due to fever being diagnosed with infective endocarditis with advanced plu- rivalvular involvement by Streptococcus salivarius. Given the clinical status and the echocardiographic findings, early surgical intervention was decided, and a double valvular replacement was performed. The case is a good example to review the echocardiographic findings associated with endocarditis.
