Endomyocardial fibrosis belongs to the group of restrictive cardiomyopathies. Although it is a rare condition, it might have a progressive course, with limited treatment possibilities and finally, can progress to end-stage heart failure. In our case we present a 71-year-old man, with a progressive course to an advanced heart failure and we show how a multi-imaging approach was a cornerstone for the diagnosis, understanding the pathophysiology and the mechanisms involved in this disease.
