We present a a case of a 53-year-old female with a history of lumbar surgery who is admitted to the hospital several times for heart failure refractory to diuretic treatment. Different imaging techniques were able to help determine the type of heart failure and the etiology of it. In this case, the diagnosis of arteriovenous fistula (AVF) could be reached as a treatable cause of high-output heart failure.
