We present the case of a 19-year-old patient who, after detecting a heart murmur on physical examination, was diagnosed with hypertrophic cardiomyopathy on echocardiography. He maintains preserved left ventricular systolic function, but very low Global Longitudinal Strain values. Cardiac magnetic resonance imaging shows the presence of extensive late gadolinium enhancement and also identifies a Cor Triatriatum that had gone unnoticed by echocardiography. A high risk of sudden death was calculated and it was decided to implant an automatic defibrillator.
