Patent foramen ovale (PFO) is a remnant of the embryological circulation present in 25% of the adult remnant found in 25% of adults that can cause intracardiac shunt and produce paradoxical embolisms. The clinical presentation as myocardial infartion is uncommon, and clinical suspicion should be raised in patients with a low burden of traditional cardiovascular risk factors, once other sources of embolism have been ruled out. Therapeutical approach should be individualized on a case-by-case basis.
