Isolated anterior mitral valve cleft (not associated with atrioventricular septal defect or endocardial cushion fusion
defect) is a rare cause of congenital mitral regurgitation. Whenever possible, mitral valve repair (direct cleft suture
with or without prosthetic annulus insertion) is preferable instead of valve replacement. We present a clinical case
in which we describe the usefulness of three-dimensional (3D) transesophageal echocardiography (TEE) for the
diagnosis and morphological evaluation of the defect to aid in the planning of the surgical procedure.
