Point of care ultrasound: The Fifth Pillar of Physical Examination

Point-of-care ultrasound as an extension of the physical examination has become a fundamental complement in the evaluation, follow-up and treatment of our patients. The education and training of physicians is necessary to be able to use it properly. We present the case of an 86-year-old woman initially admitted for sepsis of probable biliary origin. After an exhaustive reassessment, clinical signs of a probable endovascular infectious focus were found. A bedside echocardiography showed a mobile vegetation in the native aortic valve. The transthoracic echocardiography confirmed the diagnosis of endocarditis.
