We report the case of a 70-year-old man, hypertensive, and aortic coarctation surgery 50 years ago, with effort dyspnea. Transesophageal echocardiography was performed, showing a supravalvular mitral membrane and parachute-like mitral valve, with also a tomographic study, finding a coronary anomaly, mitral membrane, and non-significant aortic recoarctation
